Shooting Room Liability Waiver

I hereby acknowledge that my child is participating in the shooting room facility at The Clinton Arena and agree to the following:

1. I understand and agree that my child must adhere to the safety guidelines and rules of the shooting room facility. These guidelines include, but are not limited to, the following: children under 15 must be accompanied by an adult. All players must be wearing a helmet, gloves, and closed-toe shoes. Damaging the room or removing items from it is prohibited, and engaging in horseplay is not allowed.

2. I understand and agree that disciplinary actions may result due to any offense against the safety guidelines and shooting room rules, including but not limited to, temporary or permanent suspension from the shooting room facility.

3. I understand and agree that I am responsible for any damages caused by my child to the shooting room facility and its equipment. Any damages ensued by a person or persons due to reckless behavior will be covered by the parents of the child/children that caused the damage.

4. I hereby release, discharge, and hold harmless Clinton Arena and Clinton Youth Hockey, its owners, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, while participating in the shooting room facility.

5. I understand that the shooting room facility is subject to monitoring at all times.

6. I hereby certify that I have read and understand this waiver and release and that I am legally competent to enter into this agreement on behalf of my child.
